Student COVID Testing at Ruth Acty

UPDATE, 4/19/21:

All students may receive free, optional, COVID testing bi-weekly at Ruth Acty.
The test administered is the mouth swab test, no nasal swab required! Young, test-averse children have reported it is no problem :) This week COVID testing will be at Ruth Acty on Thursday, April 15th.

Steps 1-3 below should be completed now. Step 4 must be completed the day of (no more than 24 hours in advance of) the test.

Step 1: ONE-TIME Registration with PMH laboratories
Register online here. Fill out all areas of the form that have an asterisk (*).
- For "Client #" use code #1405 for Ruth Acty.
- For "Covid Test Type" select "PCR" from the drop-down menu

Step 2: Uninsured Program Form (ONLY IF UNINSURED)
The District is committed to making testing available to all students. When registering with PMH in the previous step you will be asked for your insurance information. If you do not currently have health insurance, please fill out the form at the link below so the cost can be covered. This form will be required on a monthly basis. STEP 3: ONE-TIME Parent Consent Form
The form says you are consenting to have your child tested through June of 2021 and covers all testing dates until school is out. Parent/Guardian Consent Form for COVID Testing.

STEP 4: TEST DAY Test Request Form
This form must be completed by 8:15am the morning of a test and can be completed up to 24 hours before test. It must be filled out before your child takes a test each time as it asks for any underlying conditions and possible Covid-19 symptoms or exposure. This must be completed online by 8:15am on test day, or your child can bring the form to school with them. COVID-19 Patient Test Request Form.

  • Please make sure all information is completed for each form in order to receive your results and process the test accurately.

  • Results can be expected 48 to 72 hours post-test date.

  • If your student has a positive test, BUSD district nurses will reach out with next steps for your student immediately.

  • Testing documents can be completed online at the links above, or printed and filled out at home and sent with your student. We will manage all paperwork either way.
